Read More Background: Epiglottitis is an inflammatory disease involving the epiglottis, vallecula, arytenoid cartilage, and aryepiglottic wrinkles. Acute epiglottitis is relatively rare, but it can quickly lead to a life-threatening airway obstruction. Diagnosis of epiglottitis is clinical, but radiography and direct vision of the epiglottis can also help with the diagnosis. Clinical manifestations of epiglottitis include fever, sore throat, and in the event of imminent airway obstruction, symptoms of muffled sound, falls, tripod position, and stridor might be presented as well. Epiglottitis can be caused by some bacterial agents or viruses such as adenovirus and measles. COVID-19 was first detected in December 2019 in a group of patients with severe respiratory symptoms in Hubei Province, China. Common symptoms include fever, cough, fatigue, difficulty breathing, and loss of smell and taste. Although COVID-19 disease has a variety of manifestations, only one case of epiglottitis following COVID-19 disease has been reported in adults. However, no cases of epiglottitis have been reported in children with COVID-19. Case presentation: The patient, a 4-year-old boy, came to our center and the patient's family complained about shortness of breath and progressive lethargy of the child from 10 days ago. Since last week, he had been sleeping in a sitting position due to the aggravation of shortness of breath. On physical examination, respiratory distress, tachypnea, subcostal retraction, tail stridor, and crackle was heard in both lungs. Diagnosed with suspected bronchoscopy epiglottitis; and a large, swollen, and cherry-red epiglottis was observed. Based on the patient's history of respiratory symptoms and the prevalence of COVID-19, a reverse pharyngeal polymerase (RT-PCR) transcript test was requested for COVID-19 and the COVID-19 infection was positive. The patient was intubated due to decreased oxygen scorching and was treated with antibiotics due to the diagnosis of epiglottitis and recovered. Conclusion: In this article, we reported the case of a 4-year-old child with epiglottitis in the context of COVID19. To the best of our knowledge, no case of epiglottitis had been, previously, reported in children with COVID19. Therefore, in our opinion, epiglottitis can probably be one of the manifestations of COVID19 in children.

Read More Background: Natural selection such as mutations is known as a constant process for viral fitness and selective adaptation. Understanding the effects of each mutation, especially on structural proteins in the viral life cycle, is important in tracking the viruses behavior. Here, we evaluated the effects of mutations in SARS-CoV-2 nucleoprotein (N) and spike (S) genes on the protein stability, immunogenicity, and pathogenicity in Iranian COVID-19 patients from Golestan province. Methods: In this study, 8 SARS-CoV-2 RNA samples were enrolled from referral hospitals in Golestan province. These samples were confirmed using a real-time RT-PCR assay targeting the SARS-CoV-2 nucleoprotein (N) and ORF1ab genes (Pishtazteb, Iran). Next-generation sequencing (NGS) was done on samples and subsequent sequences were retrieved from Global Initiative on Sharing All Influenza Data (GISAID) EpiCoV database. Structural analysis was performed between wild type (Wuhan accession number: NC_045512.2) and mutant N and S proteins to evaluate their stability, immunogenicity, and pathogenicity via bioinformatics servers such as Dynamut, Prodigy, IEDB, and software’s (Mega XI and Pymol II.V.II visualizer). Results: Amino acid codon changes in N and S proteins show that mutations could alter the translation efficiency. Normal Mode Analysis (NMA) by Dynamut server shows that stability and flexibility are changed by the mutations of these proteins. Immunogenicity analysis indicates the potential effects of some mutations such as P681H, Q675R, L699I, and D3L on immune escape. Interaction complex binding energy and affinity are higher in the mutant type compared to the Wuhan wild type, indicating higher pathogenicity. Conclusion: The results indicate that there are some important mutations in N and S genes that affect the virus behavior in the infectivity. Regarding the sample size limitation and various mutations in SARS-CoV-2 variants, other studies using whole-genome sequencing with larger sample sizes will be required. Therefore, continuous monitoring of the SARS-CoV-2 genome seems important.

Read More Background: The COVID-19 pandemic has significantly impacted people's sports and physical activities. Many studies have examined these effects from different points of view. This study investigates the challenges of COVID-19 in sport for all, emphasizing Children's Sport and Physical Activity.Methods: This research is applied in terms of purpose and descriptive-analytical in terms of data collection, and is conducted by the use of the field method. The population included all experts in children's sports, kindergarten teachers, primary school teachers, and managers of the clubs active in children's sports in Gorgan (N = 750). The sample was determined based on the statistical population and Morgan table (n = 257). A 22-item researcher-made questionnaire with a five-point Likert scale was used to collect the data and assess the effects of COVID-19 on children's sport in three dimensions and ten components. Given the normal distribution of data, the correlated t-test was used to measure the effects. And Friedman's test was used to prioritize the challenges of COVID-19 in sport for all, emphasizing children's sports.Results: The results showed that the challenges of COVID-19 in sport for all with an emphasis on children's sports and physical activities are jeopardizing the physical health of children, increasing the issues related to holding competitions and festivals, increasing unemployment of sports coaches, increasing governmental restrictions for the operation of sports facilities, reducing investment in children's sports, endangering the physical health of individuals, loss of human resources, the financial crisis of sports centers, reducing the income of health-related sports centers and increasing psychological problems (Social Phobia, anxiety and fear of Sports).Conclusions: The results revealed that the challenges raised by COVID-19 in children's sports and physical activity can be managed by giving priority to low-risk sports in open spaces, games, and competitions without spectators, providing unemployment insurance services to children's sports activists, using technology in sports classes, providing support packages and banking facilities for sports center owners and observing health practices.

Read More Background: This study aimed to determine factors affecting compliance with Hand Hygiene (HH) from the perspective of the health care team working in the pediatric intensive care unit of a children's tertiary center, Ali -Asghar hospital, in Tehran, Iran.Methods: we recruited 35 Pediatric ICU health care workers, including nurses, attending physicians and medical students, selected by purposive sampling between February to November 2020. Data were collected using deep semi-structured interviews, and analyzed through Graneheim and Lundman’s (2004) qualitative conventional content analysis method. All interviews were recorded and typed in the MAXQDA software and then analyzed in eight steps including writing interviews, determining the semantic units, text coding, matching codes with text, categorizing and developing categories, reviewing the categories, identifying the sub themes and reporting the findings.Results: 68.5% of the participants were female, with an average age of 32.7 years and work experience of 8.6 years. After reviewing all codes obtained, 4 main categories were finally extracted. (I) Meta-organizational factors including two subcategories of macro-health policy, sanctions and economic problems, (II) Organizational factors including three subcategories of incoherence in observing the rules, monitoring and evaluation, and the prevailing attitude of the organization, (III) Individual factors including three subcategories of attitude of the health care team towards patients, skin reactions to hand sanitizers, concerns about their own health and health of their family, (IV) Characteristics of the clinical environment including three subcategories of professional relationships, special conditions governing the pediatric ICU, and care facilities.Conclusion: Overall, 234 codes were extracted from the interviews and after the integration of similarities, four key themes including Meta-organizational factors, Organizational factors, Individual factors and Characteristics of the clinical environment were obtained indicating factors affecting hand hygiene performance of the PICU health care team.
